After clicking the button in the upper-right corner, a new browser tab should open that looks like the one shown on the right.

Choose “Initial” if you are a new client, or if you are returning after a several months break with a new issue.

Schedule “follow up” if it’s been less than 6 months and it’s for the same or similar issue.

Choose one of the open time slots.
